Vital Records - Birth & Death Certificates
Citizens born in Missouri may obtain a certified copy of the birth record at our office if they were born after 1920. Prior to 1920 copies must be obtained through the Missouri Center of Health and Senior Services.
Certified Death Certificates are available from the 1985 to present day. Between the years of 1984 and 1950 you must contact the Missouri Department of Health and Senior Services. Prior to 1950 you may access unofficial copies through the Missouri Secretary of State’s Web site. For deaths prior to 1910 you may want to check the Cooper County Historical Society Website.
You may obtain a copy of the needed certificate by coming to the office and applying in person. The process usually takes about 15 minutes.
The fee for birth and death certificates is established by law.
Birth Certificate $15.00.
Death Certificate $13.00 for the first and $10.00 for each additional copy within the same purchase.
Certificates can be issued by mail. The person wishing to receive a certificate by mail must first call or request an application be faxed or mailed. When the completed application has been received, approved and contains corresponding payment, the certificate will be issued by mail.
The Cooper County Public Health Center accepts cash and checks. We regretfully are unable to accept debit or credit cards.
The Cooper County Public Health Center follows guidelines set forth by the Missouri Department of Health and Senior Services on distribution of certificates.
