Vital Records

Birth, Death, & Other Vital Records

Birth Records

Citizens born in Missouri may obtain a certified copy of the birth record at our office if they were born after 1920. Prior to 1920 copies must be obtained through the Missouri Center of Health and Senior Services.

Death Records

Certified Death Certificates are available for the years 1985 to present. Death certificates for years between 1950 and 1984 must be obtained from the Missouri Department of Health and Senior Services. Unofficial copies of certificates dated prior to 1950 can be accessed through the Missouri Secretary of State’s website. Check the Cooper County Historical Society website for information on deaths prior to 1910.

In Person

A certified copy of a birth/death certificate may be applied for in person at the CCPHC. The process usually takes about 15 minutes. At the time of application, a photo ID, Social Security card, or a piece of mail with name and address of the individual applying for the certificate is required.

Fees for Birth and Death certificates are established by law.

  • Birth Certificate $15.00 each
  • Death Certificate $14.00 for the first copy and $10.00 for each additional copy at time of purchase

Cash, check, and debit/credit card are accepted. There is an additional fee associated with debit/credit purchases.

By Mail

Certificates can be issued by mail. Call to request an application to be mailed or faxed, or click on the link below to print a copy of the certificate application. Once the completed application has been received, approved and contains the correct payment, the certificates will be issued. Only cash or checks will be accepted for requests by mail.

The Cooper County Public Health Center follows the guidelines set forth by the Missouri Department of Health and Senior Services for the distribution of certificates.
